小细胞肺癌患者免疫检查点抑制剂相关脑炎1例报告

Immune checkpoint inhibitor-associated encephalitis in a small-cell lung cancer patient

  • 摘要: 免疫检查点抑制剂(immune checkpoint inhibitor,ICI)相关神经系统毒性的发生少见,临床诊治经验不足。本文梳理1例晚期小细胞肺癌(small cell lung cancer,SCLC)老年男性的病历资料。患者在接受2个周期化疗联合免疫治疗(斯鲁利单抗,200 mg)后,出现乏力、认知障碍、运动性失用和共济失调症状,结合血神经递质和头颅磁共振等检查,诊断为ICI相关脑炎,经激素治疗后症状改善。

     

    Abstract: The neurotoxicity related to immune checkpoint inhibitors (ICI) is rare, and the clinical experience remains limited. This paper reported an elderly male patient with advanced small cell lung cancer (SCLC). Following two circles of chemotherapy combined with immunotherapy (serplulimab, 200 mg), the patient developed fatigue, cognitive impairment, motor apraxia, and ataxia. Based on the examination of blood neurotransmitters and brain magnetic resonance imaging, the patient was diagnosed with ICI-associated encephalitis, and his symptoms improved after hormone treatment.
